Understanding your insurance plan

Your medical coverage is between you and your health insurance company. You are responsible for knowing your plan details, including any copays or deductibles that apply. Before your appointment, gather your current insurance card and be prepared to provide this information to our staff. This helps us verify your coverage and process your visit smoothly.

If you have a referral requirement, your referring provider must obtain it prior to your visit. The Eye Center can answer general questions about insurance, but we recommend contacting your insurance company directly if you need specific details about your coverage or out-of-pocket costs.

When someone else pays your bill

If another person is responsible for payment through their insurance policy, they should either attend your appointment or provide us with their information. We will need their date of birth, address, phone number, and the last four digits of their social security number for billing purposes. Please also bring a current copy of their insurance card.

Medical eye care versus vision care

A medical eye care appointment addresses a specific vision complaint that requires specialized care. These visits are billed to the medical portion of your insurance and are subject to your plan’s copay, deductible, or coinsurance. The Eye Center focuses on medical eye care services and does not currently accept vision care plans.

Payment methods and self-pay options

We accept cash, check, Visa, MasterCard, and American Express. Self-pay patients receive a 30% discount at the time of service. This discount applies only when the full balance is paid during your visit. Any remaining balance forfeits the discounted rate and will be billed at full cost.

If you need to arrange a payment plan, please discuss this directly with your provider during your appointment. Our front office staff cannot make these arrangements, but your physician can work with you to find a solution that fits your situation.
